Organization History
We are a non-profit organization dedicated to protecting and preserving primary healthcare access in Alberta.
The Alberta Primary Care Clinic Owners Association (APCCOA) was founded in 2024 in response to the growing challenges faced by primary care clinic owners across Alberta. Recognizing the urgent need for advocacy and support, a group of dedicated clinic owners came together to address issues such as unsustainable business models, rising operational costs, and physician shortages. Since its inception, APCCOA has been committed to empowering clinic owners through cost-saving initiatives, mentorship, and collaboration with government bodies and stakeholders. Our mission is to ensure the long-term sustainability of primary care clinics and improve access to healthcare for all Albertans.

At APCCOA, we advocate for the interests of primary care clinic owners, operating independently from the constraints that limit the Alberta Medical Association (AMA). As clinic owners, we are uniquely positioned to negotiate from a business perspective, leveraging our collective impact to address systemic challenges in the primary care system. Together, we control the infrastructure that ensures primary care exists and thrives in Alberta.

The strength of our voice lies in our numbers—more members mean greater influence. By joining forces, we amplify our ability to drive meaningful change and advocate for sustainable solutions.
A Broken Business Model
As clinic owners, we face the harsh reality of a broken business model. The growing number of clinic closures is a clear indicator of the urgent need for reform. Primary care clinics are a cornerstone of Alberta's healthcare system, meeting 90% of patient needs. Yet, the financial and operational sustainability of these clinics is in jeopardy.
The government must recognize that sustaining and expanding primary care clinics is in their best interest. Without immediate action, the foundation of Alberta’s healthcare system will continue to erode.
Addressing the Income Divide
A significant and widening income gap exists between primary care physicians and specialists. Over the past three decades, this gap has increased by several hundred percent. Primary care physicians not only earn less but also bear higher overhead costs, exacerbating the divide. As clinic owners, we are disproportionately affected, investing our personal funds—often borrowed—to establish and maintain the infrastructure essential for primary care.
Despite these investments, operational costs have far outpaced stagnant fees. The value of our clinics has plummeted, and we, the owners, are earning less than the physicians we employ. This inequity highlights a fundamental flaw in the system.
Something is fundamentally wrong with this.
Our Call to Action
At APCCOA, we are committed to addressing these issues head-on. We advocate for fair compensation, sustainable business practices, and recognition of the critical role clinic owners play in Alberta’s healthcare system. Together, we will work to ensure that primary care clinics remain viable, capable of expansion, and equipped to provide high-quality care for all Albertans.

Dr. Andrew Kohler, the founder of APCCOA, is a visionary leader with over 36 years of experience as the owner of Callingwood Crossing Medical Clinic, a legacy in Alberta’s healthcare community. A dedicated advocate, Dr. Kohler has been working tirelessly with the government for years to address the challenges faced by clinic owners and to ensure sustainable primary care for Albertans.

Dr. Don Wilson, the president of APCCOA, is a dedicated leader and passionate advocate for primary care. As the owner of Hawkstone Medical Clinic in Edmonton, he brings firsthand experience and insight into the challenges faced by clinic owners. Dr. Wilson is committed to strengthening Alberta’s primary care system through innovative solutions, collaboration, and advocacy.
